Dependant Visa

Service description

Dependant Visas are issued for family members of beneficiaries of diplomatic, official, courtesy, business and student visas, subject to the type of visa granted to the main family member or employer on whom they depend.

Requirements

1. Visa Form filled and completed in type or clearly printed.

2. One (1) frontal picture size 2 x 2 inches, with a white background.

3. Original Passport valid for at least the duration of the requested visa or longer.

4. Marriage Certificate and Canadian Birth Certificate (SHORT FORM -including parents' name) of children. This document must be notarized, authenticated at Foreign Affairs Canada in Ottawa, translated into Spanish and legalized.

*All Canadian documents that require legalization through our Embassy must be authenticated by Global Affairs Canada.

*Once you receive your document authenticated by Global Affairs Canada, you may send them all via mail/post to our Embassy.

5. Copy of the Dominican Visa or Residence Card and copy of the Passport of the person on whom the applicant depends.

6. Medical Certificate showing a diagnosis of the overall health of the applicant, indicating if he or she suffers from any contagious disease. It must be issued by the health authorities of the country in which the person resides at the time of application. This document must be notarized, authenticated at Foreign Affairs Canada, translated into Spanish and legalized.

7. Criminal Record Certificate issued by the appropriate authorities of the country in which the person resides at the time of application. This document must be notarized, authenticated at Foreign Affairs Canada in Ottawa, translated into Spanish and legalized. If the applicant has previously resided in the Dominican Republic, the document must be issued by the Attorney General's Office. Not required for minors.

8. In the case of minors, permission of the parents or guardian when the applicant is not the mother or the father. If the child is not to be accompanied into the country by both parents, it is required that the parent who is not in the Dominican Republic authorizes the other parent to move with the child permanently to the Dominican Republic. This document must be notarized, authenticated at Foreign Affairs Canada, translated into Spanish and legalized.

9. Legible photocopy of National Identity Document of the country of nationality and legible photocopy of the Residence Card if residing in a second country.

10. Documents showing the applicant’s financial solvency (Letter of Bank, Letter of Employment, titles of property). If a document is issued in the Dominican Republic, must be certified by a notary public and legalized by the Attorney General’s Office (Procuraduría General de la República).

11. Legible photocopy of former Dominican Visas or Residence Card (if renewal).

12. Application Letter from the beneficiary addressed to the consular Section including name, nationality, place of residence and occupation as well as information on reasons for seeking to obtain the visa ( of Dominican origin, child of a Dominican, Dominican spouse, as pensioner, or investor).

 

Procedure to follow

Send the documents specified in the requirements section and the completed application form to our Embassy.

Monday - Thursday 9:30 AM – 3:30 PM / Friday 9:30 AM - 1:30 PM

US$ 150.00 It is NON-REFUNDABLE, so please make sure you comply with all the requirements, before you submit your application. Call our Consular Section for more details.

Hours of service

10- 15 business days, depending on date of travel and prior requests.

Service channel

In person or via mail (Canada Post, FedEx, DHL, etc.): you must sign and place fingerprints on the application form in the presence of a notary public in your area, making sure that the stamp and the signature of the notary do not cover your signature or fingerprints.
